RELEASE OF CLAIMS AND WAIVER OF LIABILITY

To: Yukon Home And Tour, Yukon Territory, Canada

Upon Yukon Home And Tour accepting my registration for the tour, I, the undersigned, _________________________________________________________ (full name) agree to this “Release of Claims and Waiver of Liability” form and hereby understand:

A. That I am fully responsible for my own well-being and physical condition while taking part in any of Yukon Home And Tour trips and I am responsible to use appropriate clothing for the arctic winter conditions during winter trips and activities.

B. That insurance coverage is the responsibility of each individual participant & I am responsible for my own purchase of cancellation and travel coverage for health and accident insurance, etc.

C. That this document, signed and dated, must be received by Yukon Home And Tour before I can depart for the trip booked.

Furthermore, the undersigned agrees and declares that:

 I accept all of the risks including the possibility of death, personal injury, property damage and loss resulting from the trip with Yukon Home And Tour. I release Yukon Home And Tour, its employees, guides, agents and representatives from all liability for any personal injury, death, property damage or loss I may suffer as a result of my participation in the trip.

 I certify that I am physically capable and fit to participate in all activities and I have no medical conditions or needs other than those listed in my application form provided to Yukon Home And Tour

 I hereby agree to permit other participants of the trip to film or take photographs of my participation in the tour and that these images might be used for advertising purposes.

I confirm that I am eighteen years of age or older and that I have read and understand this agreement before signing. Participants under 18 years old must have a parent or guardian read and sign this document on their behalf.

I agree that the laws of the Yukon Territory govern this contract and that any legal concerns will be handled solely in the competent and fair courts in Whitehorse, Yukon, Canada.
